The DS-2019 (Formerly IAP-66) Request Form must be approved by a University of Houston department. Consequently, its use is limited to the University of Houston faculty and staff only. It must be signed by the appropriate persons indicated on the form. Once completed it must be submitted to our office with the appropriate service fee. Others are free to download the request form for informational purposes only.

The DS-2019 request form is in Portable Document Format (PDF). You will need to have Adobe Acrobat Reader Installed on your computer to download the form and fill it out online. After filling out the form online, print the form, get the necessary signatures, and bring the completed form to our office with the applicable fee.
